Эффективность раствора АСН по Бокерия—Болдыреву при хирургическом лечении взрослых пациентов с обструктивной гипертрофической кардиомиопатией
Изучена эффективность фармакохолодовой кардиоплегии растворами повышенной буферной ёмкости при хирургическом лечении обструктивной гипертрофической кардиомиопатии у взрослых пациентов (N=51), оперированных в период 2013-2018 гг. В 1-й группе (N=28) кардиоплегию проводили раствором НТК (Кустодиол), во 2-й (N=23) — раствором АСН по Бокерия—Болдыреву. Средний, минимальный и максимальный возраст в 1-й группе составил 46, 21 и 64 года, во 2-й — 42, 14 и 70 лет. В обеих группах выполняли миоэктомию выводного отдела левого желудочка по Бокерия—Морроу, а также аннулопластику трикуспидального клапана и пластику (или протезирование) митрального клапана. При фибрилляции предсердий осуществляли криогенное воздействие температурой -60°С на устья правых и левых лёгочных вен. Операции выполняли в гипотермическом режиме при 28°С. Достоверные межгрупповые различия отмечали в латентном периоде асистолии после начала антеградного или ретроградного введения кардиоплегического раствора. В 1-й группе при антеградном или ретроградном введении латентный период асистолии составил 53±7 или 170±30 с соответственно, во 2-й — 32±6 и 97±11 с (р<0.0001). Среднее время пережатия аорты в 1-й и 2-й группах составило 66±8 и 64±6 мин, после снятия зажима электромеханическая деятельность сердца восстанавливалась через 35±7 и 30±6 с соответственно. Биохимические и клинические данные не выявили достоверных межгрупповых различий. Кардиоплегический раствор АСН по Бокерия—Болдыреву обеспечил эффективную защиту миокарда от интраоперационной ишемии у взрослых пациентов с обструктивной гипертрофической кардиомиопатией, достоверно снижая латентный период асистолии.

Effectiveness of Bokeria—Boldyrev ACH solution in surgery of adult patients with obstructive hypertrophic cardiomyopathy
The study examined effectiveness of pharmaco-cold cardioplegia (PCCP) employing solutions of enhanced buffer capacity during surgery of obstructive hypertrophic cardiomyopathy in adult patients (n=51) operated on during 2013-2016. In group I, PCCP was performed with НТК (Custodiol) solution, whereas in the second group it was carried out with Bokeria—Boldyrev ACH solution. The mean, minimal, and maximal ages in group I were correspondingly 46, 21, and 64 yr, whereas the respective ages in group II were 42, 14, and 70 yr. All the patients were subjected to myoectomy of the exit pathway in the left ventricle according to Bokeria-Morrow procedure and annuloplasty of tricuspid valve accompanied with reconstructive surgery or mitral valve replacement. The atrial fibrillation (if any) were suppressed with cryoablation of the openings of the right and left pulmonary veins at -60°С. In both groups, the surgery was carried out in hypothermic mode at 28oC. The significant intergroup differences were observed in the asystole latency after antegrade or retrograde injection of cardioplegic solution. In НТК group, the values of asystole latency determined after antegrade or retrograde injection were 53±7 or 170±30 sec, correspondingly; in АСН group, the respective data were 32±6 and 97±11 sec. The intergroup differences were significant (р<0.0001). The mean time of aortal clumping in НТК and АСН groups were correspondingly 66±8 and 64±6 min (р>0.05). After removal of aortic cross-clump, the electromechanical action of the heart in НТК and АСН groups restored in 35±7 and 30±6 sec, respectively (р>0.7). In these groups, the lengths of stay in ICU were 23±2 h and 24±2 h, correspondingly (р>0.4). Durations of APV were 16±3 h and 17±3 h, respectively (р>0.7). After termination of artificial circulation, the catecholamine indices in НТК and АСН groups were 11±6 and 9±6 µg/kg/min, correspondingly (р>0.5). There were no fatal outcomes in both groups. The biochemical and clinical data revealed no significant intergroup differences. Overall, the novel Bokeria—Boldyrev ACH cardioplegic solution provides effective protection of the myocardium against intraoperative ischemia in adult patients with obstructive hypertrophic cardiomyopathy.
